Every assessment answers a single question: what impact did X have on Y? You write both in your own words, and they appear inside every question the respondent reads. Reverse them later to measure the other direction.
Define your own dimensions, lenses, questions, weights and rating bands — or adapt one from the library. Author in Excel, import, publish. A published version is frozen, so last year's numbers still mean what they meant.
A private link per respondent, one open anonymous link, or a facilitated workshop where a panel agrees each score together. Group results stay hidden below a minimum size, so nobody can be identified.
Attach evidence and moderate any score, with the reason recorded. The report shows the respondent score and the moderated one side by side, so the adjustment is part of the argument rather than hidden in it.
